Health as a right of all and the duty of the state to guarantee it according to the Federal Constitution, it has been operated by the Unified Health System, where the model of basic health care has been improved. Thus, in relation to the expansion of newly created municipalities such as Pescara Brava, the problem arises as are the budgetary conditions in basic health care in these municipalities? With the following objective to present and characterize the budgetary behavior, as well as to the execution of the budget between the revenues and the expenses, realized in small municipalities newly created, with very few inhabitants. In order to reach the goal a descriptive research is needed, which describes the per capita application of current expenditures on health in the local government with a case study. Finally, it is possible to see a low application of current expenditures as a basic pharmacy - distribution of drugs worth $ 28.95, with a higher average in the State of Pernambuco, which is $ 17.93. It is necessary to strengthen the autonomy of local governments, proportional to the needs of the expenses, since the amounts are higher with personnel than with the need of consumables
